§ 19-279. Application.

  • (a)

    Application for an arena or auditorium license shall be made to the unified government business license department.

    (b)

    Any person making application for an arena or auditorium license shall make a verified application in writing on a form provided by the business license department and shall pay to the unified government the license fee and any applicable occupational tax. Such application shall further require the following information:

    (1)

    The name, age and residence of the applicant and a statement describing the period of time in which the applicant has been in business. If a nonresident partnership or sole proprietor, the applicant shall state the name and address of its resident agent for the purpose of accepting service. If a corporation, the applicant shall state the name, address of the principal business office, state of incorporation and the name and address of the chief officer of the corporation; if a foreign corporation, the applicant shall state the name, address of the principal business office, state of incorporation, the name and address of the chief officer of the corporation, and the name and address of the corporation's resident agent.

    (2)

    The exact location of the premises to be licensed.

    (3)

    The name of the business and a description of its organization and ownership.

    (4)

    A description of the nature of events to be held at or upon the licensed premises.

    (5)

    A description of the steps the licensee shall take to ensure the safety of attendees and the general public with regard to the operation of the arena or auditorium and the events held thereon.

    (6)

    A statement of the general nature of the business.

    (7)

    Any additional information as the license administrator may require.
