§ 34-48. Duplicate receipt procedure.

The license administrator will issue a duplicate occupation tax receipt to replace any occupation tax receipt previously issued which has been lost, stolen, defaced, or destroyed without any willful conduct on the part of the person to whom it was issued. The person applying for the duplicate receipt must file an affidavit sworn to before a notary public of this state attesting to the facts surrounding the loss of the original receipt and must pay a fee of $5.00.
