§ 30-189. Application.

  • A health department permit required by this division shall be issued upon a written application filed with and approved by the health officer. Such application shall contain the following information at a minimum:

    (1)

    The name of the applicant;

    (2)

    The address of the applicant;

    (3)

    The phone number of the applicant;

    (4)

    The license tag information for the vehicle;

    (5)

    The types of wastewaters to be hauled;

    (6)

    The tank capacity; and

    (7)

    Other information deemed necessary by the health officer.
