§ 37-128. Application.

  • Every person wishing to drive a taxicab or livery vehicle on the streets of the city shall file with the license administrator a written application for a certificate to be known as a driver's certificate, or proof that such person has been issued a driver's certificate issued by any other city, by any state or by any political subdivision with standards comparable to those set forth in this article. The application shall be made upon forms furnished by the license administrator and contain such information as may be required, including the name, age, residence, sex, height, weight, color of eyes and hair, nationality, place of birth, whether a citizen of the United States, length of time the person has resided in the metropolitan area, whether the person has ever been convicted of a violation of a federal or state law or a city ordinance, whether the person has previously been authorized by any city or state as a taxicab or livery vehicle driver, and if so, where and whether such authority was ever revoked, the cause, an such other information as may be required. Each applicant shall file with the application two copies of a photograph of the applicant taken within three months of the date of the application. The size of such photographs shall be determined by the license administrator. Every applicant shall submit with the application a signed release authorizing the license administrator to have access to such criminal history record information as the license administrator may lawfully demand and have access to.
